Family Practice - Geriatrics Physician

Job Setting:

PACE (Program of All-Inclusive Care for the Elderly) - Medical Director role providing comprehensive medical and supportive services to seniors with chronic care needs in the community and at the adult day health center

Types of Cases:

Frail and elderly population aged 55+; participants certified to need nursing home care but living safely in the community; chronic disease management; geriatric care; comprehensive medical assessments including history and physicals, periodic reassessments, physical examinations, laboratory and diagnostic tests

Minimum Requirements

Board Certified/Board Eligible:

Board Certified in Internal Medicine or Family Practice (Board Eligible NOT accepted)

Certifications:

DEA Registration; CDS Certification; Board Certification in Geriatrics highly preferred

Licensure:

Current and active plenary physician license to practice in North Carolina

Experience:

Minimum of one (1) year of documented experience working with a frail or elderly population; previous supervisory experience; effective skills in physical assessment and chronic disease management for frail older adults
