This is a 13-week contract position.

Schedule

Day shift, 5x8 hours, Monday to Friday from 8:30 AM to 5:00 PM, totaling 40 hours per week.

Job Description

The Registered Nurse / Case Manager will focus on emergency department and medical inpatient case management coverage, coordinating discharge planning and patient transitions in collaboration with social work. This role requires the ability to manage medically complex patients and adapt to varied patient needs across multiple units.

Responsibilities

Coordination of discharge planning for adult and geriatric patients.
Working in partnership with social work to ensure effective patient transitions.
Adapting to coverage needs across different inpatient units as required.
Maintaining patient care standards and managing clinical complexities in case management.

Required Experience / Certifications / Licensure

Minimum 2 years of hospital inpatient RN case management/discharge planning experience.
Experience in Med-Surg and/or Critical Care discharge planning.
Active Maryland RN license required at the time of submission.
BLS certification required.
