POSITION OVERVIEW\n

Harbor Health is seeking a Board-Certified Family or Internal Medicine Physician to join our innovative, team-based care environment at our Round Rock Northeast location in Round Rock, TX. In this role, you will deliver primary care and preventive services as part of a care team actively redesigning how we approach healthcare — focusing on the health outcomes of your panel, member experience, and waste reduction rather than traditional productivity metrics. POSITION DUTIES & RESPONSIBILITIES\n

• \n

  Deliver high-quality, evidence-based primary and preventive care through in-person, virtual, and telephone visits

\n
• \n

  Manage a panel of members with focus on population health and individual member outcomes

\n
• \n

  Collaborate with interdisciplinary care team members to design and implement member-centered care protocols

\n
• \n

  Participate in continuous quality improvement initiatives to enhance clinical outcomes and member experience

\n
• \n

  Maintain accurate clinical documentation and engage with data-driven insights to inform clinical decision-making

\n
• \n

  Contribute to the development and refinement of clinical workflows and care processes within a team-based model

\n
• \n

  Engage in professional development and evidence-based practice discussions with team members

\n
• \n

  Document and code all member contacts accurately and legibly in the electronic medical record

\n
• \n

  Function as an active member of the medical team and participate in care team meetings

\n
• \n

  Contribute to organizational initiatives including clinical committees, education, quality review, and other mission-aligned activities

\n

DESIRED PROFESSIONAL SKILLS & EXPERIENCERequired:\n

\n
• \n

  Current Texas medical license or ability to obtain one

\n
• \n

  Board certification in Family Medicine or Internal Medicine

\n
• \n

  Current DEA and CPR certification

\n
• \n

  Valid Texas Driver's License

\n
• \n

  Ability to meet and maintain current organizational credentialing and privileging requirements

\n
• \n

  Strong clinical judgment and excellent diagnostic skills

\n
• \n

  Proven track record of delivering high-quality, patient-centered care

\n
• \n

  Excellent communication and interpersonal skills

\n
• \n

  Computer literacy with ability to navigate electronic health record systems

\n
• \n

  Ability to interact effectively with persons from diverse backgrounds

\n
• \n

  Sound judgment, flexibility to shift priorities, and commitment to confidentiality

\n

Preferred:\n

\n
• \n

  Experience with or interest in value-based care models

\n
• \n

  Familiarity with electronic health records and health data analytics

\n
• \n

  Experience collaborating in team-based care environments

\n
• \n

  Interest in population health and preventive care

\n
• \n

  Bilingual in English/Spanish

\n
