Staff - Registered Nurse (RN) - Case Manager, Hospice - $72K-105K per year

Job Title Registered Nurse (RN) Case Manager, Hospice – Full‑time – Beverly, Massachusetts. Overview Gentiva Hospice seeks an experienced RN to lead compassionate hospice care. The RN Case Manager coordinates skilled nursing care, collaborates with interdisciplinary teams, and supports patients, families, and caregivers throughout the hospice journey. Responsibilities Act as primary point of contact for patients and families, guiding them through hospice care. Deliver and document skilled nursing care based on each patient’s individualized Plan of Care; visit homes, assisted living, or other settings. Perform initial and ongoing assessments to monitor patient condition and adjust care. Collaborate with physicians, LPNs, CNAs, social workers, chaplains, and others to build and refine personalized care plans. Provide emotional, physical, and spiritual support to patients and loved ones, including education, comfort, and bereavement guidance. Teach caregivers safe nursing techniques and offer emotional reassurance. Communicate patient condition updates to the care team to ensure timely interventions. Maintain accurate clinical documentation and follow infection control and compliance standards. Support quality improvement and scheduling initiatives to ensure consistent, compassionate care. Help patients and families navigate medication administration, hospice philosophy, symptom/pain management, and end‑of‑life processes. Essential Functions Perform comprehensive patient assessments and reassessments, including in‑person and telephonic RN screenings. Deliver skilled nursing interventions and symptom management aligned with goals of care. Develop and update individualized care plans, including visit frequency, and identify risks. Coordinate care across interdisciplinary teams, consulting with physicians and nurse partitioners. Educate patients, families, and caregivers about disease process, self‑care, end‑of‑life care, medications, nutrition, and dietary needs. Communicate changes in condition or signs of decline to appropriate providers. Support continuity of care by recognizing and facilitating appropriate referrals and transitions. Document timely clinical records, including screening/assessment findings, physician orders, progress notes, and care plans. Maintain effective relationships with IDG and involve the team when needed. Participate in on‑call and patient support activities as required. Minimum one year of RN experience in acute care, oncology, long‑term care, hospice, or home health. Preferred hospice or palliative care experience. Current CPR certification. Valid driver’s license and automobile insurance. Excellent assessment, documentation, communication, and program management skills. Knowledge of terminally ill patients, hospice services, managed care principles, and regulatory guidelines (Medicare, Medicaid, ACHC). Ability to perform multiple tasks efficiently and with composure. Work Conditions Patient homes, assisted living facilities, or other settings; travel by automobile; potential exposure to infections, bodily fluids, and hazardous materials. Location Beverly, Massachusetts (Gentiva Hospice). Compensation Salary range: $78,595 – $98,270 per year. Benefits Competitive pay and 401(k) with company match. Medical, dental, vision insurance.
