What You'll Do:
- Provide patient-centered physical therapy services to promote mobility and functional independence
- Develop and implement individualized treatment plans based on patient assessments
- Document therapy sessions accurately in EMR Meditech Home Care
- Monitor patient progress and adjust therapies as needed
- Educate patients and caregivers on therapy exercises and home programs
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes
- Travel within a 50-mile radius to serve multiple clients efficiently
What You Need:
Required:
- Valid Physical Therapist license in Louisiana
- BLS certification
- Minimum 1 year of acute care or outpatient experience
- Strong knowledge of rehabilitation techniques
- Good communication and organizational skills
Preferred:
- Experience with home health settings
- Familiarity with EMR systems and infusion therapies
About the Assignment:
