Job Description

Job Title: RN-Homecare/Hospice
Location: West Point, NE
Duration: 13 weeks
Schedule: Variable - Days

Travel Gross weekly: $2250 – 2350/week (Based on Exp)

Local Pay: $57-60/hr (Based on Exp)

Job Description:

RN needed ASAP for homecare/hospice 30 mile radius from West Point, NE. Will visit 3-5 homes per day. Vehicles and gas are provided to our travelers by the facility Mon- Fri with every 4th weekend on callResponsibilities

Provide skilled nursing care to patients in their homes.
Provide hospice and end-of-life care as appropriate.
Assess patient conditions and develop/implement individualized care plans.
Administer medications and treatments as ordered.
Monitor patient progress and communicate changes to the healthcare team.
Educate patients and families regarding medications, treatment plans, and disease management.
Maintain accurate and timely clinical documentation.
Coordinate care with physicians, therapists, aides, and other healthcare professionals.
Respond appropriately to patient needs during scheduled on-call periods.
Follow all applicable nursing, home health, hospice, and facility policies.

Requirements

Active RN license eligible to practice in Nebraska.
Previous home health and/or hospice nursing experience preferred/required.
Strong clinical assessment and documentation skills.
Ability to work independently and manage multiple home visits.
Excellent communication and interpersonal skills.
Valid driver's license and ability to travel to patient homes.
Ability to work Monday–Friday and participate in every 4th weekend on-call rotation.
Must be available to start ASAP.
