Responsibilities
- Deliver skilled nursing care in patients' homes.
- Coordinate care plans with providers and families.
- Provide education and emotional support.
- Complete accurate clinical documentation.
- Promote comfort and quality of life.
Qualifications
- Active Oklahoma RN license.
- Home Health or Hospice experience preferred.
- Excellent communication and time-management skills.
- Valid driver's license and reliable transportation.
