Position Summary
Under the direction of the Social Work Supervisor, the Medical Social Worker provides psychosocial assessment, individualized care planning, supportive counseling, advocacy, referral, and case-management services for participants of Family Bridges’ Community-Based Adult Services centers. Working on site as part of the interdisciplinary team, the Medical Social Worker supports participants and families in addressing psychosocial needs, accessing community resources, and maintaining continuity of care.
Essential Duties & Responsibilities

• Screen prospective participants and communicate with participants, family members, caregivers, referral sources, and agency representatives regarding eligibility, needs, and the intake process.
• Complete timely and comprehensive psychosocial intake assessments, including required home visits, and identify participant strengths, needs, risks, supports, and service preferences.
• Develop, implement, monitor, and update individualized psychosocial care plans in collaboration with participants, families, caregivers, and the interdisciplinary team.
• Participate actively in interdisciplinary team conferences and communicate psychosocial findings, changes, barriers, risks, and recommendations that affect participant care and treatment planning.
• Provide ongoing case management, reassessment, service coordination, follow-up, and discharge or transition planning consistent with participant needs and program requirements.
• Provide individual and family supportive counseling within the scope of the position and refer matters requiring specialized clinical, legal, protective, or emergency intervention to appropriate resources.
• Connect participants and families with appropriate community services, benefits, agencies, health care providers, and facilities; serve as a liaison and advocate to support access and continuity of care.
• Maintain complete, accurate, timely, and confidential case-management and medical-record documentation, including initial assessments, progress notes, care plans, reassessments, referrals, outcomes, and ongoing evaluations.
• Complete required administrative and program responsibilities, including statistical records, reports, medical records, meetings, audits, quality-improvement activities, and other assigned documentation.
• Conduct home visits and local community visits using a personal vehicle as required and follow applicable safety, mileage, and participant-confidentiality procedures.
• Provide culturally and linguistically responsive services and communicate with participants and families in English and Cantonese and/or Mandarin, as required by the assignment.
• Recognize and promptly report suspected abuse, neglect, exploitation, safety concerns, significant changes in condition, or other matters requiring mandated reporting, clinical review, or supervisory intervention.
• Perform other related duties as assigned by the Social Work Supervisor or Program Director.

Minimum Qualifications & Competencies

• Master of Social Work degree from a program accredited by the Council on Social Work Education required.
• Entry-level candidates and candidates with four or more years of social work experience may be considered; experience in human services, health care, community-based care, or geriatric services is strongly preferred.
• Experience working with frail older adults, adults with disabilities, families, and interdisciplinary care teams is preferred.
• Thorough knowledge of social work principles and practices, including psychosocial assessment, case management, care planning, supportive counseling, crisis response, advocacy, and community resources for older adults and their families.
• Strong ability to assess participant and family needs, establish professional relationships, exercise sound judgment, and work effectively with interdisciplinary team members, community organizations, and the public.
• Strong written and verbal communication skills and ability to prepare clear, complete, accurate, and concise assessments, care plans, case notes, correspondence, and reports.
• Bilingual proficiency in English and Cantonese and/or Mandarin is required. Proficiency in Vietnamese, Filipino/Tagalog, Korean, or another Southeast Asian language is desirable.
• Valid California Driver’s License, a driving record acceptable to the agency’s insurer, and reliable access to a vehicle for required home and community visits.
• Ability to use electronic health records, computers, and standard office applications and to maintain participant privacy and confidentiality.

Working Conditions
Community-Based Adult Services, office, participant-home, and community environments with frequent participant and family interaction, interdisciplinary meetings, documentation, computer work, and local driving. The position may encounter medical, behavioral, family, crisis, or environmental conditions typical of health care and home-visit work. Employment Conditions

• Work Location: This is a full-time, on-site position. Local home and community visits are required as part of assigned duties.
• Orientation Period: This non-exempt position includes a three-month orientation period beginning on the employee’s first day of employment.
• Health and Compliance Requirements: Employment is subject to completion and maintenance of health screenings, background clearances, training, certifications, driving requirements, and other qualifications required by law, regulation, funding source, insurer, or Family Bridges policy.

• Ability to travel independently to participants’ homes, community locations, and Family Bridges sites and safely navigate varied indoor and outdoor environments.
• Ability to lift and carry files, supplies, or equipment weighing up to 20 lbs., with or without reasonable accommodation.
