We are seeking a compassionate and skilled MDS Nurse to join our dedicated team of healthcare professionals. About the Role:

• Conduct thorough initial and ongoing assessments of residents' physical, emotional, and social needs, using the MDS process to identify and prioritize their care plans.

• Develop and implement individualized care plans, collaborating with the interdisciplinary team to ensure comprehensive and personalized care.

• Manage and coordinate the delivery of resident care, ensuring that all necessary services and treatments are provided in a timely and effective manner.

• Monitor and document resident progress, making adjustments to care plans as needed to ensure optimal outcomes.

• Provide education and support to residents, family members, and caregivers, promoting self-advocacy and independence.

• Identify and report any changes in resident condition, contributing to the development of interventions and strategies to prevent complications.

Responsibilities:
