Join our dedicated team at Skilled Nursing Pharmacy in Hayward, CA as a PM Pharmacist, where you'll play a vital role in delivering exceptional pharmaceutical care to skilled nursing facility patients. - Dispense and verify medications accurately for skilled nursing facility (SNF) patients during PM shift hours
- Perform drug utilization reviews (DUR) and ensure compliance with long-term care pharmacy regulations
- Process and verify new medication orders, refills, and stat orders in a timely manner
- Collaborate with nursing facility staff and healthcare providers to resolve medication-related issues
- Monitor for drug interactions, contraindications, and therapeutic duplications
- Supervise and guide pharmacy technicians during shift operations
- Ensure proper documentation and adherence to state and federal pharmacy regulations
- Active California Pharmacist license in good standing
- Experience in long-term care, skilled nursing, or institutional pharmacy preferred
- Proficiency with pharmacy management software and dispensing systems
- Strong knowledge of geriatric pharmacotherapy and SNF medication protocols
- Excellent attention to detail and ability to work efficiently in a fast-paced environment
- Strong communication skills for collaborating with interdisciplinary healthcare teams
- Ability to work PM shift hours, including potential weekends or holidays
