Schedule: Monday - Friday, 8am-4pm; hours may vary
Job Description:
Education: Minimum: High School Diploma or GED/HSE accredited by the State Board of Education Preferred: Associates degree in social work or related field. Peer Support Professional Certification preferred.
Experience: Minimum: 1 year of experience in social work or healthcare field Preferred: 2+ years of experience in social work or healthcare field
License/Certification: Peer Support Professional Certification required within 90 days of hire
Job Duties:
1. Coordinate logistical supports and circumvent barriers to treatment for patients
2. Care Management and Resource Utilization
3. Community Outreach and Education
