We are seeking a compassionate and dependable Physical Therapist Assistant to join a SNF in Memphis. This permanent position involves working with post-acute patients recovering from surgery, illness, injury, and other medically complex conditions. Under the supervision of a licensed Physical Therapist, you will provide hands-on rehabilitative care focused on improving mobility, strength, endurance, and overall functional independence. Job Responsibilities:

• Assist the Physical Therapist in implementing individualized treatment plans for post-acute patients

• Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions

• Monitor patient progress and communicate any changes or concerns to the supervising Physical Therapist

• Educate patients and caregivers on prescribed exercises, safety precautions, and mobility techniques

• Maintain accurate and timely documentation of treatments and patient progress

• Ensure compliance with facility policies, safety standards, and regulatory requirements

• Active state license as a Physical Therapist Assistant in Tennessee

• Current BLS/CPR certification

• Minimum of 6 months of recent PTA experience within the last 5 years preferred

• Experience in post-acute, SNF, or rehabilitation settings is a plus
