HealthRIGHT 360 gives hope, builds health, and changes lives for people in need by providing comprehensive, integrated, compassionate care that includes primary medical care, mental health services, and substance use disorder treatment.
The Integrated Care Center Clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, mental illness, and chronic medical conditions. We provide team-based care in an integrated care setting.
The Advanced Practice Provider Residency is a two-year program with HealthRIGHT 360. During the 2 years of training, the residents will be engaged in a diversity of learning opportunities including didactics, preceptorships, and specialty rotations. Residents will start with an intensive boot camp to shore up clinical basics as well as EMR training, an orientation to practice management and team-based care, and basics of Quality Improvement, as well as other topics. The APP Resident will participate in weekly didactics, begin with shadowing a preceptor and build their own practice panel with preceptor support.
Specialty rotations will include those that support care for our safety net population, including Addiction Medicine, Mobile & Street Medicine, Women's Health, Psychiatry, Behavioral Health, HIV care, and Hepatitis C care. In the second year of the residency, residents will continue to receive training and preceptorship to continue to enhance their clinical practice. Second year residents will also serve as mentors for first year residents. Cohorts of residents will consist of a minimum of two members. Incentives will be offered for relocation and retention.
Job Summary
The Advanced Practice Provider Residency Program Director (APP Residency Director) is responsible for the design, implementation, and evaluation of the structure, content, curriculum, and outcomes of the APP Residency Program. Clinically, the Director supervises, mentors, and evaluates residents throughout the duration of the program and collaborates closely with clinical preceptors to ensure a consistent, high-quality learning environment.
The APP Residency Program focuses on primary care services delivered in a safety-net, community health center setting and includes collaborative rotations with specialty services such as Addiction Medicine, Mobile and Street Medicine, Women's Health, Psychiatry, Behavioral Health, HIV Medicine, and Hepatitis C care. The Director provides coaching and support related to caring for safety-net populations, particularly patients experiencing homelessness and those with complex co-occurring conditions, including substance use disorders, mental illness, and multiple chronic medical conditions.
In addition to residency leadership responsibilities, the Director maintains a clinical panel of patients in the clinic. The size of the panel will reflect protected time to ensure successful fulfillment of residency program responsibilities. This leadership role includes participation in key organizational meetings related to clinic operations, population health, funding, research, and quality improvement. The Director also leads the Residency Advisory Committee (RAC) and regularly presents updates on program progress, outcomes, and challenges.
Key Responsibilities
Residency Responsibilities:
- The Director is an expert primary care Nurse Practitioner (FNP or AGNP) or Physician Assistant (PA-C) who integrates direct patient care responsibilities with clinical leadership and program oversight.
- Provide leadership in the recruitment, orientation, curriculum development, and evaluation of all program components, including didactic education, case presentations, and resident assessment.
- Facilitate high-quality teaching and learning through didactic sessions, case facilitation, feedback, coaching, and support for both residents and preceptors.
- Provide and coordinate participation of internal staff and external subject matter experts in didactic sessions, precepting, and other educational activities.
- Ensure resident progress through direct precepting and support of specialty preceptors across all rotations.
- Review and respond to resident reflective journals on a rotating basis in collaboration with other program leaders.
- Promote the APP Residency Program's model, reputation, and impact through writing, research, presentations, and policy-related activities as assigned by HealthRIGHT 360, including engagement with current and potential funders.
- Coordinate outside electives and trainings, including hospital applications, verification of malpractice coverage, and evaluation processes.
- Distribute, collect, analyze, and synthesize resident, rotation, and faculty evaluations, providing actionable feedback for residents and preceptors and ensuring appropriate follow-up and reporting.
- Utilize evaluation data to support ongoing enhancement and continuous improvement of the APP Residency Program.
- Coordinate the partnership with the UC Davis APP Fellowship Program, including didactic participation, communication with faculty and residents, scheduling and coordination of in-person rotations, evaluation processes, and site visits.
- Serve as a primary information resource regarding the residency program for internal teams, partner institutions, and the broader community.
- Actively participate in the APP Consortium to share best practices, build professional networks, and strengthen the APP Residency Program.
- Serve as the primary preceptor for APP residents during their primary care rotations and continue mentorship throughout specialty rotations.
- Coordinate all specialty rotations in collaboration with specialty providers.
- Develop and manage the APP Residency Program clinical schedule in partnership with the Clinic Leadership and preceptors.
Leadership Responsibilities:
- Collaborate with the Chief Healthcare Officer (CHO), Medical Directors, Operations Director, and other organizational leaders to define, implement, and evaluate policies related to the APP Residency Program.
- Champion and actively participate in quality improvement initiatives for both the APP Residency Program and the clinic.
- Coordinate and oversee the general orientation of new residents, including department, clinical areas, and institutional onboarding.
- Provide direct supervision of APP residents and residency staff, including hiring, performance evaluation, ongoing supervision, and, when necessary, termination.
- Active member of clinics leadership team contributing to the overall success of the clinics.
- Participates in budget process including maximizing visit targets, creating and adjusting program budget.
Clinical Care Responsibilities:
- Provide high-quality, patient-centered care consistent with professional licensure, credentialing, and privileging.
- Complete timely, thorough, and accurate clinical documentation.
- Demonstrate competency in all essential procedures and clinical skills within the scope of practice.
- Order, prescribe, recommend, and interpret diagnostic tests, treatments, and therapies as appropriate.
- Prescribe medications and non-pharmacologic treatments within scope of practice.
- Provide expert consultation within the practice and across the interdisciplinary care team.
- Facilitate and promote evidence-based practice to support patients in achieving individualized health goals.
- In the event a resident leaves the APP Residency Program prior to completing the two-year commitment, assume additional patient care responsibilities to offset reduced clinical capacity.
Administrative Responsibilities:
- Maintain compliance with HealthRIGHT 360 policies, procedures, and clinical standards to ensure high-quality care and adherence to HRSA, managed care, and other applicable standards.
- Actively participate in grant writing and reporting activities to support program sustainability.
- Collaborate with the philanthropy and development team to share program outcomes and successes.
- Complete all required Relias and other mandatory trainings on schedule.
Education and Experience
Required:
- Master's or Doctorate in Nursing (FNP or AGNP); Master's or Doctorate in Science (Physician Assistant, PA-C).
- Possession of a current, valid California Registered Nurse License; and Possession of a California Nurse Practitioner Certificate; OR
- Possession of a current, valid California License to practice as a Physician Assistant issued by the California Board of Medical Quality Assurance
- BLS certification
- Clinical experience with and commitment to working with diverse client populations including the homeless, persons with substance abuse and mental health issues.
- Experience: Minimum 2-3 years of experience as a practicing licensed Nurse Practitioner or Physician Assistant and 2 years of experience as an APP lead, manager or preceptor or other equivalent academic teaching experience.
- Evidence of progressive leadership experience
