Licensed Practical Nurse (LPN) – Home Health | Americare We are seeking a Licensed Practical Nurse (LPN) to provide high‑quality care in a home health setting, supporting patients in Americare communities and in‑home environments. Position Summary This role focuses on patient‑centered care, coordination with care teams, and maintaining a safe, comfortable, and home‑like environment that supports each patient’s physical, emotional, and social well‑being. What You’ll Do Provide direct patient care in home health and community‑based settings Serve as a liaison between Americare Home Health and community teams Assess patient needs and support individualized care plans Implement nursing standards, policies, and care protocols Monitor patient condition and report changes to the care team Collaborate with physicians, nurses, therapists, and leadership Provide guidance and support to caregivers and nursing staff Ensure accurate documentation and compliance with regulations What We’re Looking For Basic knowledge of home health or senior living care Strong attention to detail and clinical accuracy Ability to work independently and manage priorities Strong communication and teamwork skills Commitment to maintaining patient confidentiality and dignity Qualifications Graduate of an approved Practical Nursing (LPN) program Current, active LPN license in the applicable state Maintain required state certifications and continuing education Previous experience in home health, long‑term care, or senior living preferred Join Americare Home Health as an LPN and make a difference—one patient, one home, one day at a time. #J-18808-Ljbffr
