Responsibilities
- Conduct comprehensive physical therapy assessments for residents and develop individualized treatment plans.
- Implement therapy interventions to improve mobility, strength, balance, gait, posture, and functional independence.
- Monitor progress, re-evaluate goals, and communicate changes to the interdisciplinary team.
- Document evaluations, treatment plans, progress notes, and outcomes in compliance with regulatory and facility requirements.
- Support discharge planning, caregiver education, and strategies to promote long-term functional gains.

Requirements
- Active Physical Therapist license in NH or eligible for licensure in New Hampshire.
- Experience in or demonstrated ability to work within a skilled nursing/geriatric population.
- Availability for a 13-week travel assignment with guaranteed hours.
- Strong clinical documentation and communication skills.
- Ability to work on-site in Laconia, NH and adhere to a 08:00–16:30 shift schedule (5 days per week).
