Growing together.The Level2 Registered Nurse delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of type 2 diabetes. The Registered Nurse is an integral part of the interprofessional care team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life. The Registered Nurse supports and advocates for people affected by type 2 diabetes to optimize quality care. The Registered Nurse promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.*** Candidate must be available to work Monday-Friday 8:00 am - 5:00 pm with at least one day per week 10:00 am - 7:00 pm. ***If you reside within a compact state, you will have the flexibility to work remotely* as you take on some tough challenges.Primary Responsibilities:Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practiceAdvocates for and communicates about improved quality of care and outcomes for those living with type 2 diabetesActively participates in the quality improvement process and adapts practice/process based on quality findingsPartners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetesApplies the AADE7 self-care behaviors to educate on and initiate behavior changeEngages in lifelong learning and serves as a role model of professionalismProvides quality diabetes self-management education via telehealth visits based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with diabetesFollows the National Standards of Diabetes Self-Management and Support GuidelinesCompletes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient' assessed needs and goals and promotes successful self-managementDocuments all individual contacts/visits in the Electronic Health Record and outcomes data base according to the guidelines in a timely mannerCollaborates, advocates, and confers other members of the Level2 care team in developing person-centered diabetes plansEvaluates and analyzes continuous glucose monitor data to guide diabetes education and supportContributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulationsParticipates in meetings, serves on committees, and represents the Level2 nursing department within the enterprisePerforms other duties as assignedYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current and unrestricted RN compact licenseCertified Diabetes Care and Education Specialist1+ years of outpatient diabetes self-management education and support experiencePreferred Qualifications:Working knowledge of diabetes technology/continuous glucose monitorsProven excellent verbal and written communication skills.Proven basic computer skills including use of Microsoft OfficeProven self-sufficient troubleshooting technology problems and working independentlyProven highly organized and strong attention to detailProven solid problem solving and critical thinking skillsProven ability to work with a diverse patient population and have exceptional customer service skillsPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $60,200 - $107,400 annually based on full-time employment.
