Registered Nurse - Hospice

Job Summary:


Seeking a compassionate and dedicated Registered Nurse to provide hospice care services, delivering patient-centered support and clinical expertise in home settings. Responsible for managing a balanced caseload while ensuring compliance with all regulatory and agency requirements.

Responsibilities:

• Administer hospice care visits, including initial, follow-up, and wound care assessments.
• Document patient interactions, clinical findings, and interventions accurately within Homecare Homebase (HCHB).
• Coordinate with the care team to develop and update individualized care plans.
• Travel up to 75 miles daily to provide timely and efficient patient care across assigned territories.
• Participate in periodic office meetings and team collaborations to ensure quality service delivery.
• Complete all documentation within 24 hours to support billing and continuity of care.

Qualifications:

• Hold an active nursing license in the state of assignment, with no exceptions.
• Have a minimum of one year of recent hospice or adult home health experience.
• Proficient in Homecare Homebase (HCHB) and recent use within the past six months.
• Possess current CPR and BLS certifications.
• Willing to travel up to 75 miles and accept a flexible schedule including on-call rotations.
• Reliable transportation, valid driver’s license, and liability insurance required.
