The City of Alamosa is in Alamosa County, Colorado, United States. The city population was 8,780 at the 2010 United States Census. The city is the commercial center of the San Luis Valley in south-central Colorado, and is the home of Adams State University. Alamosa was established in May 1878 by the Denver and Rio Grande Railroad and quickly became an important rail center. The railroad had an extensive construction, repair and shipping facility in Alamosa for many years and headquartered its remaining narrow gauge service here with trackage reaching many points throughout southwest Colorado and northern New Mexico. Alamosa is now a notable tourist town with many nearby attractions, including the Great Sand Dunes National Park and Preserve, Colorado Gators Reptile Park and the Rio Grande Scenic Railroad. The town hosts "Summer Fest on the Rio" which occurs the first weekend in June, the Early Iron car show over the Labor Day weekend, and "Weekends on the Rio" on various Sundays throughout the summer. The city takes its name from the Spanish adjective Alamosa, meaning "of cottonwood", for the cottonwood forests which grow along the Rio Grande and throughout town. • Documenting observations, assessments, and changes in patient's condition.
• Collaborating with healthcare team members and other interdisciplinary teams to facilitate positive patient care outcomes.
• Providing a full range of complex and comprehensive nursing care to patients with a variety of physical and/or behavioral problems.
• Accountable for meeting and maintaining the qualification standards and functional statements of the job.
• Demonstrates organizational skills which enable completion of work in a timely manner with demonstrated ability to assume a leadership role in a patient-centered program.
• Establishes effective communication with patients, families, visitors and members of the interdisciplinary team.
• Prepare patients for surgical and diagnostic procedures and assist with procedures as necessary.
• Conduct initial assessment on patients; develop nursing care plans based on identified patient needs, coordinate with ancillary services to assure provision of care, document all plans, assessments and care provided. Provide patient and family education throughout the hospitalization. Initiate discharge planning.
• Document observations, assessments, and changes in patients' condition.
• Collaborate with health team members to facilitate positive patient care outcomes.

QUALIFICATIONS

• Diploma, Associate Degree or Bachelor of Science in Nursing (preferred) from an accredited program, college, or university.
• Hold a current license to practice as a Registered Nurse in any State, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
• Has been practicing actively as an RN for at least the last two years.
• Capable of providing complex and comprehensive nursing care for adult medical and surgical patients.
• Current Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) Certifications.
• Has documented training and skills verification.
• Has been actively practicing as a registered nurse in primary care for two years.
