In-Home Palliative Care RN Case Manager

ProHealth Care is seeking a dedicated RN Case Manager to work collaboratively with patients and healthcare professionals in a home care/hospice setting. The RN will assess, plan, facilitate, and evaluate high-quality care for patients in their homes, providing autonomy and flexibility. Ideal candidates will hold an Associate’s Degree in Nursing and a valid RN license. This role requires strong communication skills, negotiation skills, and experience in both pre-acute and post-acute care.
