Registered Nurse – Home Health
Schedule: 8 hours per shift
Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40 hours per week
Requirements:
- Current state license as a Registered Nurse
- CPR certification
- Homecare Homebase Software experience
- OASIS experience
- 1-2 years' experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- CHPN certification (preferred)
- Reliable transportation and insurance
Roles & Responsibilities:
- Assess home health patients to identify physical, psychosocial, and environmental needs.
- Develop and evaluate the Plan of Care (POC) for quality and continuity of care.
- Educate patients and families on care, medications, and safety.
- Supervise and provide clinical direction to home health aides.
- Monitor assigned cases to ensure compliance with third-party payer requirements.
- Provide effective communication among patients, families, and healthcare team members.
