Overview Physical Therapist – General, Barre, VT. Start date 08/31/2026 for a 13-week travel assignment with guaranteed 36 hours per week. Weekly pay range: $2,329–$2,420. Shift is Tuesday–Saturday, 8:00 a.m.–5:00 p.m. Setting: Home healthcare - hospice. This is a travel temporary assignment through Healthforce Healthcare LLC. Key Highlights Weekly pay: $2,329–$2,420 per week Guaranteed hours: 36 per week Contract duration: 13 weeks Shift: Tuesday–Saturday, 8:00 a.m.–5:00 p.m. Start date: 08/31/2026 Setting: Home healthcare - hospice Ongoing credentialing and clinical support during the assignment Responsibilities Provide one-on-one physical therapy in patients’ home settings, focusing on goals and independence Assess condition and develop individualized plans of care to maximize mobility and functional outcomes Collaborate with an interdisciplinary team to ensure patient needs are met Conduct standard therapy measurements (ROM, strength, balance) and apply a range of modalities Perform OASIS-based documentation and case management; maintain accurate EMR records Requirements Active Vermont physical therapy licensure Home health experience with OASIS documentation required EMR experience preferred (McKesson/Netsmart strongly preferred) Ability to travel to patients’ homes within the service area Availability for the full 13-week assignment with no planned time off Benefits and Support Guaranteed hours: 36 per week Travel assignment support and dedicated recruiter assistance Credentialing and onboarding support to facilitate a timely start Ongoing clinical and scheduling support throughout the assignment
