Physical Therapist (PT) - Home Health

We are seeking a dedicated Physical Therapist (PT) specializing in Home Health to join our team. The primary responsibility is to provide direct patient care services, ensuring high-quality rehabilitation support in clients' homes. This role involves collaboration with healthcare teammates and active participation in the patient's comprehensive care plan.

Shift Notes

This position consists of 8-hour workdays, with a guarantee of 40 hours per week.

Weekend and Holiday Requirements

• Rotating weekends and holiday coverage may be required as needed.

On-Call Requirements

• No on-call duties are necessary for this position.

Key Responsibilities

• Deliver physical therapy services to clients in their homes.
• Collaborate with interdisciplinary team members to develop and implement a comprehensive plan of care for patients.
• Utilize rehabilitation equipment such as splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities.

Qualifications

• Home Health Certification is required.
• Minimum of 2 years of acute nursing care and home care experience is required.
• Ability to work independently with minimal supervision.
• Proficient in OASIS documentation, which is essential and non-negotiable.
• A valid driver's license and a personal vehicle are required.
• Current Massachusetts Physical Therapist License must be included in the profile.
• Active American Heart Association BLS certification is mandatory.
• Completion of a skills checklist current within the past year is required.
• A signed supervisory reference from within the past two years is necessary.

Additional Information

Travelers must ensure that their permanent address is included in their profile. Any requests for time off must be noted at the time of submission, and no more than 5 days of requested time off will be accepted. Orientation for this position is non-billable and will last for 8 hours. Additionally, all personnel must be able to pass a fit test for utilizing an N95 mask, as PAPRs are not available for home health situations.
