Temp Director of Care Coordination

Job Description

We are seeking a qualified Temp Director of Care Coordination to lead interdisciplinary clinical programs, manage staff, and ensure regulatory and accreditation readiness within an acute healthcare setting.

Responsibilities

• Lead and manage case management, care coordination, and utilization management functions.
• Oversee performance improvement initiatives within the care coordination team.
• Ensure compliance with regulatory standards and accreditation requirements.
• Collaborate with clinical and operational leadership to enhance patient progression and discharge planning processes.
• Develop and implement interdisciplinary clinical programs to improve patient care outcomes.

Required Experience / Certifications / Licensure

• BSN required; a master's degree in nursing, health administration, or business administration is preferred.
• Case management certification (e.g., CCM or ACM) is preferred.
• A minimum of five (5) years of experience in an acute healthcare setting is required.
• At least five (5) years of progressively responsible leadership/administrative experience in case management, care coordination, or a closely related field is required.
• Advanced knowledge of care coordination and transitions of care in an acute-care environment is essential.

Schedule

This position involves a day shift from Monday to Friday.

Assignment Duration

This is a temporary position with a contract length of approximately 13 weeks.
