Job Description Clinical Intake Coordinator, LPN Schedule & Location Hybrid role Home Health and Hospice Monday through Friday, 9 a.m. to 5:30 p.m. Rotating on-call; 2-5 shifts every 4-6 weeks; remote Holiday rotation Missouri and Kansas LPN License required Responsibilities The Clinical Intake Coordinator facilitates the Home Health and Hospice referral process by receiving referrals from multiple referral sources within and without the Saint Luke's Health System. The coordinator determines what additional information is needed from referral sources, communicates acceptance or decline to varied referral sources, and processes accepted referrals. The coordinator prioritizes and triages hospice referrals and ensures regulatory compliance during the initial screening and intake process. Employees are required to work on-site 3-4 days per week, with the option to work remotely 1-2 days per week upon completion of orientation. Remote days are scheduled on a rotating basis to ensure adequate office coverage.
