Candidates should have at least 2 years of case management experience. Experience must include 6 months of discharge planning case management in an acute care hospital. Preference will be given to candidates with a background in large organizations or teaching hospitals. The shift will typically occur Monday through Friday. Work hours may vary between four 10-hour shifts or five 8-hour shifts based on operational needs.
