What You Will Do:

• Serve as a trusted clinical resource for hospital staff, patients, and families who are considering hospice care and need clear, compassionate guidance.
• Collaborate with physicians, case managers, discharge planners, social workers, and interdisciplinary care teams to identify appropriate hospice referrals.
• Complete timely patient assessments, review medical records, and help determine eligibility and level-of-care needs using sound nursing judgment.
• Educate patients and loved ones about hospice philosophy, available services, goals of care, and the transition process with empathy and professionalism.
• Coordinate effective handoffs that support continuity of care, timely admissions, and a positive patient and family experience.
• Use your acute care, medical-surgical, telemetry, and patient assessment background to recognize changing clinical needs and advocate for appropriate interventions.

What You Bring:

• You hold an active Registered Nurse license and bring a strong commitment to patient-centered, compassionate care.
• You have a Bachelor of Science in Nursing or comparable nursing education that supports confident clinical decision-making.
• You bring acute care, medical-surgical, skilled nursing, or hospital nursing experience and can assess complex patient needs effectively.
• You communicate clearly with patients, families, physicians, and multidisciplinary teams during sensitive conversations and transitions.
• You are organized, dependable, and skilled at care planning, medication administration, vital-sign monitoring, IV therapy, and clinical documentation.

Preferred Experience:

• Experience as a hospice liaison, hospital liaison nurse, hospice admissions RN, RN case manager, discharge planner, or transitions-of-care nurse is valued.
• Experience with trauma, PACU, dialysis, labor and delivery, or other high-acuity nursing environments is welcomed.
• Certifications such as Basic Life Support, NIH Stroke Scale, Trauma Nursing Core Course, or Neonatal Resuscitation Program can strengthen your application.

Compensation and Benefits:

• This position offers an annual salary of $85,000 to $95,000, based on relevant experience and qualifications.
• Eligible team members receive health insurance, vacation leave, sick leave, holiday pay, and paid time off.
