Registered Nurse Case ManagerShift InformationMonday to Friday, 08:00 AM to 05:00 PMMinimum RequirementsExperience: 1 year of Case Management experience in a clinic or hospital setting required within the last 2 years.Licensure: Active Registered Nursing license in Alaska or another U.S. state required.Education: Graduate of an accredited school of nursing; a Bachelor's degree is preferred.Certifications: Basic Life Support (BLS) certification is required; Case Management certification is preferred.Job DescriptionPatient CareConduct comprehensive patient assessments and develop individualized plans of care.Implement and evaluate nursing interventions and patient responses.Administer medications and treatments as per provider orders and policies.Recognize changes in patient condition and initiate appropriate interventions or escalation of care.Provide patient and family education concerning care, medications, and discharge needs.Deliver culturally responsive and patient-centered care.Nursing Judgment & Clinical Decision-MakingApply critical thinking and professional judgment to perform and interpret comprehensive assessments.Exercise independent nursing judgment in determining appropriate actions, interventions, and priorities.Develop, implement, evaluate, and revise individualized nursing plans based on patient needs.Integrate clinical findings with ethical principles, cultural considerations, and safety standards.Accept accountability for nursing decisions and outcomes in accordance with regulations and policies.Care Coordination & DocumentationCollaborate with providers and interdisciplinary team members to coordinate care effectively.Facilitate referrals, follow-up care, and continuity of services.Document assessments, interventions, and outcomes accurately and in a timely manner in the electronic health record.Advocate for patient care, assisting patients in navigating the healthcare system.Quality, Safety & CompliancePractice within nursing scope of practice, adhering to regulatory, safety, and infection prevention requirements.Participate in quality and performance improvement activities.Maintain required competencies and certifications.Report adverse events and unsafe conditions according to policy.Teamwork & Professional PracticeDemonstrate professionalism, accountability, flexibility, and teamwork.Provide guidance and delegation to ancillary staff as appropriate.Support orientation, mentoring, and education of staff, students, and peers.Be willing to assist in other departments or assignments as needed.Client Details Address 222 Tongass Ave City Sitka State AK Zip Code 99835
