• Conduct thorough and accurate Minimum Data Set (MDS) assessments for residents, identifying their individual needs and identifying areas for improvement

• Develop and implement comprehensive care plans, collaborating with interdisciplinary teams to ensure resident-centered care

• Utilize MDS data to identify trends and areas for quality improvement, contributing to the development of effective care strategies

• Provide education and training to staff on MDS processes and best practices

• Maintain accurate and up-to-date records of resident assessments, care plans, and progress
