About the Job Outpatient Therapist | The Pavilion at Williamsburg Place | Hampton, Virginia Purpose Statement: The Therapist III is a part of the multidisciplinary team responsible for providing direct clinical services to patients and their families by diagnosing and treating mental and emotional disorders, whether cognitive, affective, or behavioral. The role includes completing psychosocial assessments; providing individual, group, and family therapy; facilitating psycho‑educational training, family meetings, and discharge planning; assisting in program development; developing, implementing, and reviewing treatment plans; and providing linkage to outside resources. Roles and Responsibilities Conduct a psychosocial assessment and develop an initial discharge plan. Develop and implement treatment plans based on assessment, coordinating additional services as needed. Conduct individual, group, or family sessions as appropriate. Apply psychotherapeutic techniques and interventions to treat diagnosed emotional and behavioral disorders. Participate in multidisciplinary treatment planning conferences and present patient information to the team. Maintain an assigned caseload. Develop and implement special patient groups or program modules. Prepare progress notes and treatment updates in a timely manner, maintaining clinical documentation per regulatory and accrediting standards. Ensure each assigned patient has a structured and clinically appropriate post‑hospitalization care plan; act as liaison to referral sources and support continuity of care. Provide crisis intervention to patients and families in acute distress. Introduce and initially coordinate a wide range of community resources (financial, housing, legal, social/recreational, occupational, spiritual, and support groups). Provide case management, discharge planning, or after‑care planning according to the treatment plan. Assist in educating and training staff on behavior management or therapeutic crisis intervention. Coordinate various clinical, psychosocial, or educational activities with patients and their families. Evaluate patient needs and determine if referrals to other programs or facilities are required. Assist in training new clinical staff and interns. Education / Experience / Skill Requirements Master’s Degree in Social Work, Counseling, or Psychology (required). At least four years of experience as a therapist, with a minimum of two years in the specific population of the facility. Clinical interviewing and assessment using the Diagnostic Statistical Manual of Mental Disorders. Ability to effectively assess, plan, and implement therapeutic social work services in a multidisciplinary setting. Skill in assessment and a solid knowledge of social work principles, practice, and government eligibility for social welfare programs; ability to mobilize and coordinate resources effectively. Licenses / Designations / Certifications State or national licensure for professional therapy providers (required). CPR and de‑escalation and restraint certification (required; training available upon hire). First aid may be required based on state or facility.
