Our journey dates back to 2012 when we embarked on a transformative path as a part of PAHM. Through dedication and unwavering commitment, we evolved into an independent powerhouse in 2020, establishing a strong foundation as a trusted provider of various healthcare services. Our spectrum of care encompasses a wide array of offerings, including specialized addiction treatment programs, IDD services catering to individuals with intellectual and developmental disabilities, and top-notch nursing services. With each passing year, OneWell Healthcare has strived to enhance the well-being of our patients, and our story stands as a testament to our enduring mission of providing exceptional and comprehensive care to all those we serve. Seeing a need for a new mission and vision for those in the IDD community, PAHM made the decision to separate Supports Coordination and IDD Services into separate entities in 2021. Starting in Pennsylvania, OneWell has since expanded into both California and New Jersey to provide exceptional services in their communities. Services We Offer in NJ Community-Based Supports Providing direct support and assistance for Individuals with Intellectual and Developmental Disabilities (IDD) to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community. Individual Supports OneWell Direct Support Professionals work closely with individuals, families, and program specialists to ensure all individuals live their lives as they choose. They work 1:1 with individuals with Intellectual and Developmental Disabilities (IDD) to meet the goals set for them by their collaborative care teams. Respite Care Respite care provides short-term relief for family/primary caregivers of individuals with Intellectual and Developmental Disabilities (IDD). Respite can be arranged for an afternoon or for several days or weeks. Care can be provided in the participant's home, or in a host family home. Prevocational Training OneWell Prevocational training assists individuals with IDD in developing good work habits and gaining the basic skills and abilities essential for employment in any field. Supported Employment OneWell Supported Employment Programs help individuals with Intellectual and Developmental Disabilities (IDD) in obtaining and maintaining employment.
