About the Role As a Registered Nurse with Synergy HomeCare, you will play a vital role in developing, implementing, and overseeing client care plans. You will ensure each client receives safe, personalized, and high-quality care tailored to their needs — supporting independence while prioritizing safety and dignity. Key Responsibilities Conduct in-home client assessments and reassessments Develop, implement, and update individualized care plans Provide clinical oversight and recommendations for non-medical and personal care services Educate and train caregivers on client-specific needs and best practices Communicate regularly with clients, families, and office staff Ensure compliance with state regulations and agency standards Support quality assurance and documentation requirements Qualifications Active RN license (in good standing) Home care, geriatric, or community health experience preferred Strong assessment and documentation skills Excellent communication and interpersonal abilities Ability to work independently while collaborating with a supportive team What We Offer Competitive pay: $40–$50 per hour Flexible scheduling Paid orientation and ongoing training Time-and-a-half pay for overtime and holidays Direct deposit Employee recognition programs Supportive, team-centered culture At Synergy HomeCare, we create a world of care at home for all — serving clients of all ages and abilities.
