PRN Deliver skilled care where patients are most comfortable-at home. We're looking for a compassionate and dependable Licensed Practical Nurse (LPN) who enjoys providing one-on-one patient care in a home health setting. This is an opportunity to build meaningful relationships with patients while delivering quality nursing care that helps them recover, manage chronic conditions, and remain safely at home. What You'll Do

What You'll Do
- Provide skilled nursing care under the direction of a Registered Nurse and physician
- Administer medications and treatments as ordered
- Monitor patient conditions and report changes to the supervising RN
- Reinforce patient and caregiver education on medications, treatments, and disease management
- Assist with developing and following individualized plans of care
- Complete timely and accurate clinical documentation
- Collaborate with physicians, caregivers, and the interdisciplinary care team
- Provide personalized, one-on-one patient care
- Build lasting relationships with patients and their families
- Enjoy greater independence throughout your day
- Be part of a supportive clinical team focused on exceptional patient outcomes
- Make a meaningful impact by helping patients remain safe and independent at home
- Active LPN/LVN license (state of practice)
- Previous home health, hospice, long-term care, or clinical nursing experience preferred
- Strong clinical, communication, and organizational skills
- Ability to work independently while managing multiple patients
- Valid driver's license and reliable transportation
- Compassionate and patient-centered
- Dependable and self-motivated
- Strong attention to detail

- Committed to delivering high-quality patient care
