Job Summary

Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Coordinate care for heart transplant patients including immunosuppression and prophylactic antibiotic management.
• Monitor patients for organ rejection and infections, and conduct routine patient assessments.
• Assess graft function and health status to determine needs for further assessment or intervention.
• Coordinate medication regimen changes with healthcare professionals.
• Communicate effectively with multidisciplinary teams, patients, and families regarding care issues.
• Facilitate patient activities with consulting departments and physicians, and address educational needs.

Qualifications

• Current CA RN License and AHA BLS certification required.
• Minimum of 1 year experience in ambulatory care settings.
• Experience in cardiac nursing with management of heart failure and/or transplant patients.
• Ability to float to other facilities as needed.
• Epic charting system proficiency preferred.
• BSN required for all RN positions.
