Responsibilities

• Conduct psychosocial assessments of patients and families to identify barriers to care, discharge needs, and psychosocial risk.
• Coordinate patient care with the interdisciplinary team to support timely disposition and safe transitions.
• Provide counseling and linkage to resources for coping with illness, treatment decisions, and family communication.
• Develop and implement comprehensive care plans; document in the electronic medical record; ensure compliance with policies and regulations.
• Participate in care conferences and discharge planning meetings; advocate for patient-centered planning.

Requirements

• Active Georgia LCSW license
• AHA BLS certification
• Minimum 2 years of recent hospital-based medical social work experience
• Ability to work in a hospital-based setting in Georgia
