3 East-Acute Inpatient Rehabilitation

3 East is a 24 bed acute inpatient rehab unit. It specializes in rehabilitation of patients in order to transition home. Common diagnoses are strokes, brain injuries, transplant patients, spinal surgery, open heart surgical patients, and a variety of other diagnoses that result in debility. A team approach is applied while caring for this patient population. The 3 East Team collaborates with physical, occupational, and speech therapy.

General Overview

This job is a member of the direct care healthcare team and is responsible for providing care to patients and families under the direction of the registered nurse.

Essential Responsibilities

1. Performs general nursing care within the LPN scope of practice.
2. Assists with the preparation of equipment and assists the clinical provider during treatment, examination, and testing of patients.
3. Collects assessment data, prepares patient for examination and documents clinical information on patient record and reports any changes in patients condition to the registered nurse, advanced practice nurse or physician.
4. Orders diagnostic tests as directed by the clinical provider and gathers diagnostic tests and/or examines results prior to physician/clinical provider assessment.
5. Schedules procedures and obtains authorizations, as necessary, under the direction/oversight of the registered nurse. Assists nursing and medical staff with basic medical tasks such as administering medications, blood and tissue collection and/or other laboratory specimens; prepare patients for testing.
6. Other duties as assigned or requested.

Qualifications

Required

• High School/GED
• Licensed Practical Nurse (LPN) OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC)
• CPR American Heart Association
• Act 34 Criminal Background Clearance Certificate
• Act 33 Child Abuse Clearance Certificate
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Preferred

• 1 year in healthcare
• PALS or ACLS
• State approved IV course certification.
• Knowledge of EPIC documentation and assessment process
