Job Description for Case Manager:

Reports to and is supervised by the Deputy Administrator to support recovery-oriented services for individuals with serious mental illness transitioning from the criminal justice system into the community, in alignment with court expectations, program criteria, and state requirements.

Pay Grade

Salary commensurate with experience and education.

 

Minimum Qualifications

• Three (3) years of professional experience in the mental health field (e.g., psychology, clinical social work, mental health counseling, or a closely related discipline); and
• Master’s degree from an accredited college or university in sociology, social work/social welfare, psychology, or a closely related field; or
• An equivalent combination of related education and professional experience.
• References, licensure, or certifications as required for the position.
• Proof of education, training, and/or experience is required.

 

Duties and Responsibilities

• Coordinate with Tennessee Department of Mental Health & Substance Abuse Services (TDMHSAS) personnel to ensure program eligibility, compliance, and adherence to state and court requirements; participate in required network meetings and trainings.
• Establish and maintain collaborative working relationships with local criminal justice agencies, community-based service providers, and behavioral health organizations.
• Identify, screen, and conduct clinical assessments of individuals with mental illness who are justice-involved or incarcerated.
• Facilitate continuity of care during incarceration, ensuring timely access to behavioral health services and coordination with correctional and custodial settings.
• Develop, implement, and monitor individualized, recovery-oriented treatment plans in collaboration with interdisciplinary teams.
• Coordinate referrals, service linkages, and transitions of care throughout program participation and during community reentry or program completion.
• Collaborate with clients and family members to support recovery goals and reduce risk of recidivism, hospitalization, or crisis service utilization.
• Advocate for individuals with mental illness or those at risk of incarceration, ensuring protection of client rights, confidentiality, and informed participation.
• Participate in weekly multidisciplinary staffing meetings with the Public Defender and Assistant District Attorney teams.
• Provide education and training to promote understanding of client needs, rights, and recovery-focused practices among stakeholders.
• Conduct follow-up and ongoing monitoring to support successful reintegration into the community following incarceration.
• Perform other related duties as assigned or directed.

• Conducts accurate client assessments and evaluations. 
• Familiar with community resources and services for mental health support. 
• Understand diverse backgrounds and provides inclusive services. 
• Proficient in using electronic health records and relevant software.

 

Special Requirements: 

· Requires work in both court and community settings while maintaining strict confidentiality standards. Must successfully pass a background check and be eligible to enter secure facilities, including local jails, to conduct assessments. A valid driver’s license and reliable transportation are required to support local community outreach and service coordination, as well as out-of-city travel for required meetings, trainings, and professional development. Attendance at mandatory meetings and trainings is expected. Position also requires participation in a rotating on-call and holiday coverage schedule to support program operations and emergent needs. Must pass required Program Certification.

 

Physical Requirements: 

• Ability to sit, stand, walk, communicate effectively, use office equipment, travel within the community, and occasionally lift light materials (up to 20 pounds).
