Documentation of Patient Contact Hours (including call back hours included in the rate and charged call back hours) Physicians are required to provide supporting notes listed on their timesheet entries to verify Call Back hours (patient contact hours) submitted and associated with the acceptable occurrences mentioned above. Physicians must document in the timesheet the specific reason for the call back and they should have a description such that the note can be validated if an audit report was generated in an electronic health record. Telephone calls and medical record reviews should not be considered on site call back hours.
