JOB SUMMARY

THE RESIDENT CARE COORDINATOR (RCC) PROVIDES DIRECT CARE TO RESIDENTS AND SUPPORTS THE DIRECTOR OF NURSING IN MAINTAINING THE HIGHEST DEGREE OF QUALITY CARE. THE RCC COORDINATES THE ONBOARDING, TRAINING, SCHEDULING, AND DAY-TO-DAY OVERSIGHT OF NON-LICENSED CLINICAL STAFF, INCLUDING CAREGIVERS, CERTIFIED NURSING ASSISTANTS, AND MEDICATION TECHNICIANS. THIS ROLE MODELS HIGH STANDARDS OF CUSTOMER SERVICE, PROFESSIONALISM, COMMUNICATION, AND FOLLOW-THROUGH WHILE HELPING ENSURE THAT RESIDENT NEEDS ARE ADDRESSED PROMPTLY AND RESPECTFULLY.

Primary Responsibilities

Staff Coordination, Training & Oversight

• Coordinate In-services and Assist with Ongoing Education of Non-licensed Clinical Staff.
• Participate in the Interviewing Process for New Staff.
• Create and Maintain the Schedule for Non-licensed Clinical Staff in Conjunction with the Director of Nursing.
• Train and Mentor New Clinical Staff.
• Assist in Auditing Care Tasks and Electronic Documentation.
• Participate with the Director of Nursing and Executive Director in Care Conferences, As Appropriate.
• Coordinate and Oversee the Onboarding and Training Experience for Non-licensed Clinical Staff.
• Assist the Director of Nursing with Day-to-day Communications.
• Assist with Gathering Annual, Quarterly, and Change-in-condition Documentation.
• Ensure Care Is Performed According to the Carriages Standards.
• Participate in Department Meetings As Assigned.

Resident Care & Clinical Support

• Maintain Professional Behavior While Providing Direct Care to Residents in a Manner That Ensures Dignity and Respect for Everyone Involved.
• Measure, Record, and Track Vital Signs As Assigned by the Director of Nursing.
• Clearly Communicate Important Updates and Information from Nursing Leadership Within the Nursing Department.
• Maintain the Confidentiality of All Resident Care Information, Including Protected Health Information.
• Report Known or Suspected Incidents of Unauthorized Disclosure of Resident Information to the Direct Supervisor or Executive Director Within 24 Hours.
• Inform Appropriate Staff Regarding Resident Emergencies and Hospitalizations.
• Report Pertinent Observations Regarding Residents to the Director of Nursing and/or Executive Director.

• Interact and Communicate Professionally with Personnel, Residents, Family Members, Visitors, Government Agencies and Personnel, and the Public.

• Work Various Shifts As Scheduled, Including Weekends and Holidays.

Qualifications

• High School Diploma or General Education Degree (Ged).
• Current Cna Certification, Medical Assistant Certification, or Medication Technician Certification Required.
• Current Cpr Certification Preferred.

Experience

• Minimum of Two Years of Experience in a Hospital, Nursing Care Facility, or Other Related Healthcare Facility Preferred.
• Experience Working with the Elderly Population Preferred.

• Ability to Read and Write Legibly in English.
• Ability to Speak English Effectively with Individuals, Customers, and Employees of the Organization.
• Ability to Comprehend and Perform Oral and Written Instructions and Procedures.
• Basic Personal Computer Skills and the Ability to Use Various Software Programs.
• Strong Customer Service Skills, Including Ownership, Accurate Identification of Resident Needs, Timely Follow-through, and Effective Service Recovery.
• Ability to Communicate Professionally and Maintain Confidentiality.

Physical Requirements

• Ability to Frequently Walk and Stand for Long Periods of Time.
• Ability to Frequently Stoop, Kneel, Crouch, Crawl, Push, Pull, Lift, Grasp, and Feel.
• Manual and Finger Dexterity and Eye-hand Coordination When Handling Equipment.
• Corrected Vision and Hearing Within Normal Range.
• Ability to Occasionally Lift and/or Move up to 35 Pounds.

Work Environment

• Typical Healthcare and Residential Environment with a Noise Level Normally Within a Normal Range.
• Occupational Exposure to Blood, Body Fluids, Communicable Diseases, or Other Potentially Infectious Substances (Category Iii).
• Work Schedule May Include Day, Evening, and Night Shifts, Weekends, and Rotating Holidays.
