Assess patients' home environments and identify equipment or support needs. Develop and implement individualized, physician‑approved plans of care with measurable therapy goals. Provide dysphagia therapy and recommend appropriate diet modifications in collaboration with the care team. Recommend, implement, and train patients and caregivers on augmentative and alternative communication (AAC) devices. Educate patients and families on home exercise programs to support ongoing progress. Monitor patient outcomes, modify treatment plans as needed, and ensure continuity of care. Complete timely and accurate clinical documentation in accordance with JCAHO standards. Participate in care conferences and collaborate with physicians, nurses, physical therapists, occupational therapists, and interdisciplinary team members. What We’re Looking For Master’s degree from an ASHA‑approved Speech‑Language Pathology program, including the required clinical internship, or as required by applicable laws and regulations. Meets all educational and experience requirements for the Certificate of Clinical Competence in Speech‑Language Pathology (CCC‑SLP) granted by ASHA. Active certification to practice Speech‑Language Pathology. Minimum of one year of hands‑on clinical experience. Current CPR certification maintained throughout employment. Valid driver’s license and access to a properly insured personal vehicle. Strong clinical assessment, communication, documentation, and patient education skills.
