Conducts maternal mental health screenings and systematically reviews screening data, provides appropriate responses for identified needs around social determinants of health, collaborates with all involved teams and partners to ensure that patient and family needs are met. Provides care coordination support for families experiencing difficulties getting needs met; ensures optimal patient coordination, excellent collaboration between teams, with a focus on family experience and outcomes.What You'll Bring:Minimum EducationAssociate's Degree Associate's degree in social work, nursing, education, health sciences or related field is required (Required)Bachelor's Degree Bachelor's degree in Social Work, Nursing, Education, Health Sciences or a related field is preferred (Preferred)Minimum Work Experience2 years Experience in a behavioral health or other health care setting as a member of a multidisciplinary team providing care coordination services to patients. (Required)1 year Experience working with pregnancy and/or post partum patients, preferably providing care coordination services.
