Nurse Practitioner - Advanced Cardiac Care

Position Overview

Gotham Enterprises Ltd. is actively seeking a skilled and compassionate Nurse Practitioner (NP) to join our specialized outpatient practice at the Center for Advanced Cardiac Care (CACC) in Goshen, New York. In this full-time, onsite role, you will work under the guidance of the Chief Nurse Practitioner and in collaboration with the CACC Medical Director within the Division of Cardiology. Your efforts will significantly enhance patient care and contribute to improving health outcomes in our cardiac patient population.

Key Responsibilities

As a Nurse Practitioner at CACC, your responsibilities will include:

• Collecting comprehensive patient histories and conducting targeted physical examinations to assess patient health.
• Ordering and interpreting diagnostic tests, diagnosing illnesses, and managing both acute and chronic conditions based on established guidelines.
• Collaborating autonomously and with physicians and NP peers to formulate and execute individualized care plans tailored to each patient’s specific needs.
• Communicating and educating patients and their family members on disease processes, treatment options, and management strategies outside the hospital setting.
• Facilitating hospital admissions and safe discharges in coordination with the inpatient care team, ensuring continuity of care.
• Documenting all patient interactions accurately and promptly in our electronic medical records (EMR) system for seamless tracking of patient progress.
• Engaging in ongoing quality improvement initiatives and research protocols to elevate the standard of care provided within our facility.
• Maintaining a safe, compassionate, and therapeutic environment for patients and their families.

Patient Population / Client Focus

You will primarily work with patients experiencing a range of cardiovascular conditions, including but not limited to heart failure, pulmonary arterial hypertension, and those undergoing post-heart transplant care. Your expertise will play a crucial role in managing complex cases and ensuring that each patient receives comprehensive support throughout their treatment journey.

Qualifications

Candidates for this role should possess:

• A Master’s degree in Nursing, specifically as a Nurse Practitioner, with a focus on adult care.
• Active and current New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Certification from the NP Board, with current BCLS, ACLS, DEA, and NPI certifications.

Preferred Experience

The ideal candidate will have:

• A minimum of two years of experience in a hospital or clinical setting, particularly in cardiology.
• Experience working with patients receiving advanced therapies, including outpatient intravenous medications and mechanical circulatory support.
• Proficiency in utilizing electronic medical record systems and Microsoft Office applications (Word, PowerPoint, Excel) for documentation and communication purposes.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 120,000 to 220,000, commensurate with qualifications and experience. Our comprehensive benefits package includes:
