About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Day 5x8 Hours per Shift: 08:00 - 17:00 Experience: Minimum 1 full year Adult Home Health or Hospice experience License: Active and valid nursing license required Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase EMR and OASIS Description: Provides, coordinates, and directs home nursing care according to physician's orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Degree of travel includes home visits and periodic office meetings. Must have reliable transportation, a current driver’s license, and agency-required liability insurance. Average number of patients per day is 5-8. Contractor will be assigned between 5-10 patients per day. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Must understand that there are no guaranteed hours. Clinician must hit weekly productivity points which equate to 40 hours per week. Expected travel includes up to 75 miles from the assigned care center. Mileage will be reimbursed at the IRS standard rate. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation that impacts patient care will result in a cancellation. Clinician must accept every patient on their schedule regardless of distance between appointments. Must have experience using OASIS and HomeCare HomeBase EMR within the last 6 months. Must be open to on-call rotations and weekend coverage as needed.
