Schedule:

• Full-time, Monday through Friday
• On-call responsibilities probable
• Daily structured visit expectations

Responsibilities:

• Deliver skilled nursing care and clinical interventions in patients homes
• Perform patient assessments and complete accurate, timely OASIS documentation
• Collaborate with physicians and interdisciplinary care teams to establish and implement individualized care plans
• Manage wound care, medication administration, patient and caregiver education, and disease monitoring
• Relay changes in patient condition promptly to the clinical team

Desired Qualifications:

• Active and unencumbered RN license
• Minimum of 1 year recent home health RN experience
• Proven OASIS documentation proficiency (required)
• Familiarity with HomeCare HomeBase (strongly preferred)
• Strong organizational skills for managing a structured daily visit schedule
• Reliable transportation and valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
