Responsibilities:

• Oversee the social services department, ensuring that all services are delivered in compliance with regulatory requirements and organizational standards

• Assess patients' social, emotional, and psychological needs, developing personalized care plans that address their unique requirements

• Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to ensure comprehensive care and treatment plans

• Coordinate services with community resources, such as home health care, hospice, and other support services, to ensure continuity of care

• Develop and implement policies and procedures to ensure efficient and effective delivery of social services

• Provide support and guidance to social workers and other support staff, enhancing their skills and knowledge to deliver exceptional patient care

• Foster a positive work environment, promoting teamwork, and open communication among staff members

• Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care
