PAY $50.00 - $140.00 per visit JOB TYPE W-2 Per Diem MEDICAL SPECIALTY Home Health WORK LOCATION On the road JOB DESCRIPTION Alliance Healthcare seeks a clinically experienced RN with strong documentation and charting skills, a fast learner, and excellent communication abilities. The RN will travel to patients' homes to administer services, support independent living, and maintain comprehensive care records. AVAILABILITY Patients require care 7 days a week. The RN must be able to work at least two‑three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter, one summer). Scheduling is flexible but these minimums are required. RESPONSIBILITIES Perform home health visits and administer at-home IVs. Change dressings, clean wounds, and conduct in‑depth assessments. Oversee patient treatment plans and update doctors and the agency on patient health. Document care and upload to the agency portal. Obtain and document physician orders. Initiate and regularly re‑evaluate care plans in consultation with the Clinical Team Manager. Coordinate care with the Clinical Team Manager, physicians, and other home‑care team members. Perform re‑certifications, resumption of care, transfers, and discharges as requested. Assess need for additional team services (PT, OT, ST, MSW, HHA). Conduct Home Health Aide supervisory visits as required by regulations. Demonstrate knowledge of regulatory requirements. POSITION QUALIFICATIONS 1 year of visiting RN experience in home health (required). 1 year of nursing experience (required). Current California RN license (required). BLS certification from the American Heart Association and/or the American Red Cross (required). NSO Nursing License Insurance (required).
