Job Description

LPN (Contract) – NY 13165

Pay: $1,258 – $1,420

Schedule:

Full-Time, 5x8-hour Evening shifts
Rotating shifts as needed (Days/Evenings/Nights/Weekends)
Must be able to start orientation on a Tuesday or Thursday
Every other weekend (EOW) required
Holidays required

Position Summary & Responsibilities

Provide direct and indirect nursing care to residents
Collect/report patient data and assist with care planning
Implement and evaluate care plans under RN supervision
Collaborate with interdisciplinary team
Ensure patient safety, comfort, and proper documentation

Requirements

Active NY LPN License
BLS (AHA or RQI only)
1+ year LPN experience (LTC preferred)
Ability to care for adult/geriatric patients
Strong communication and teamwork skills
