Your responsibilities will include:Conducting annual wellness visits primarily at the patient's residence, supplemented by telehealthThoroughly documenting medical histories, symptoms, and medication informationAdministering various tests and screeningsCollaborating with our Primary Care team to ensure holistic patient careEnsuring timely completion of patient records, with access to electronic medical records and portalsProviding patient education on preventive care and prescribed treatmentsMaintaining strict confidentiality in line with HIPAA regulationsThis is a part-time/per diem Nurse Practitioner opportunity (2-3 days a week, 8am-5pm schedule). This role can include in-clinic, in-home, or telehealth visits. Home visits will be schedule within a 35 mile radius of your home region.
