Registered Nurse - Ambulatory Care/Clinic/Outpatient

Job Description

Job Summary:
The Tele/Ambulatory Care RN provides comprehensive nursing care in an outpatient environment, including assessment, care coordination, patient education, and transitions of care. This role involves delivering personalized, holistic care to patients in an ambulatory setting and collaborating effectively with healthcare teams.

Responsibilities:

• Assess patient conditions and develop individualized care plans incorporating psychological, sociocultural, spiritual, economic, and lifestyle factors.
• Coordinate and deliver nursing care in an outpatient or ambulatory setting, ensuring adherence to best practices and safety protocols.
• Educate patients and their families about health conditions, treatment plans, and post-discharge care to promote understanding and compliance.
• Communicate promptly and effectively with physicians, support staff, and patients to facilitate seamless care transitions.
• Document all clinical activities accurately in compliance with facility and regulatory standards.
• Maintain certification and licensure requirements, including BLS and CA RN licensure, up-to-date at all times.

Qualifications:

• Valid California RN licensure recognized via Nursys within the last 2 weeks.
• Minimum of 2 years of recent telemetry and ACU/PACU nursing experience.
• Current BLS certification (AHA).
• Strong clinical assessment, communication, and collaboration skills.
• Ability to work rotating night shifts, including weekends, with no on-call responsibilities.
• Compliance with travel and credentialing requirements, including valid certifications and references.
