Pharmacy Technician Care Manager

As a Pharmacy Technician Care Manager, you'll play a key role in helping patients understand, access, and stay on track with their medications. You'll work side-by-side with care teams to close gaps, prevent issues, and make sure every patient gets the full benefit of their treatment.

What your day will look like:

• Review and reconcile medications to ensure accuracy across care plans
• Talk with patients about how to take their medications safely and effectively
• Check in with patients who may be struggling with adherence
• Help coordinate refills, prior authorizations, and medication delivery
• Keep patient records accurate and up to date in EHR systems
• Partner with pharmacists to flag interactions and improve therapy plans
• Follow all pharmacy regulations and patient privacy standards
• Contribute ideas to improve care quality and workflows

Requirements: What you bring to the team:

• An active Pharmacy Technician certification (PTCB or NHA)
• Experience in a pharmacy or healthcare setting
• Familiarity with medication therapy management and chronic care support
• Confidence using EHR systems and pharmacy tools
