Roles & Responsibilities
- Provide top-notch physical therapy services to residents in a skilled nursing facility
- Develop and implement individualized treatment plans to improve mobility and function
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Document patient progress and adjust treatments for optimal outcomes
- Educate patients and families on therapeutic exercises and home programs
Qualifications
- Active Physical Therapist (PT) license in Minnesota or compact eligibility
- Doctor of Physical Therapy (DPT) degree preferred
- New graduates welcome to apply - we support your growth!
- Previous SNF experience is a plus but not required
Knowledge & Skills
- Expertise in patient assessment and treatment planning
- Strong communication and teamwork abilities
- Knowledge of rehabilitation techniques for geriatric populations
- Proficiency in electronic medical records (EMR) systems
